We assist those at risk of institutionalization
by locating and putting into place services in the
home which allow elderly people to remain in their
own homes in their own communities. By working with
families and addressing needs on a case-by-case
basis, the case manager locates services, explains
services, applies for services, sees that the
services are put in place and monitors the
situation. Also, MEOC, under sub-contract from local
housing authorities, is the care coordinator for
residents of public housing. Virginia uses a Uniform
Assessment Instrument for all of its community-based
services thereby making it less cumbersome to share
information among service providers while protecting
the confidentiality of the older person. MEOC is
involved in a new web based case management system
called “No Wrong Door” which allows elderly people
and their family caregivers to enter the long term
care system at a myriad of places and tell their
stories only once.
This is a one-stop shop for seniors, persons with
disabilities, and their caregivers and can be
accessed by persons in and out of the State. Through
No Wrong Door, individuals may obtain information on
a full range of long term care options and can
receive screening for entry into long term support
services.
