Most
frail elderly people have more problems than they can handle alone
-- chronic illnesses such as heart disease, diabetes, or depression,
changes in mobility, memory loss, reduced income, uncertain
housing, isolation, and loneliness. They need help from
various providers of care -- doctors, nurses, therapists,
pharmacists, dietitians and others. In seeking help, they have
to follow complex directions, keep appointments with a shifting set
of providers, and hope that everyone is passing along vital
information. Trying to navigate the health and social services
system can be overwhelming for older people and their caregivers.
In Mountain Empire
PACE, each participant has a team of providers to help them get the
health and social services they need. This team includes a
doctor, nurses, therapists, and social workers. The team
assesses the needs of each participant, creates a plan of care and
makes sure that each participant gets what is included in their
plan.
