The interdisciplinary Team works closely with PACE participants, family members, and caregivers to create a unique plan of care for each participant.  The plan of care outlines what services each participant needs to stay as healthy as possible for as long as possible.  Each plan of care is updated every six months or when there's a significant change in participant health status.
